WAIVER AND RELEASE AGREEMENT
Please read carefully before signing
This is a release of liability and waiver of certain legal rights.
I understand that this document is a release of liability AND IS MY LEGAL
PROMISE NOT TO SUE I understand that by signing this “Release of
Liability” I am giving up substantial legal rights to sue Born To Be Wild
Adventures its leaders, officers, director, employees, agents, sponsors and
volunteers for negligence.
I understand the risks and hazards. I understand that these activities may require
good physical conditioning. I believe I have that good physical conditioning
necessary for me to engage in these activities safely. I understand that I have
responsibilities. My participation in this activity is purely voluntary. No one is
forcing me to participate and I elect to participate in spite of the risks. I AM
VOLUNTARILY PARTICIPATING WITH FULL KNOWLEDGE OF THE
INHERENT RISKS, HAZARDS, AND DANGERS INVOLVED AND HEREBY
ASSUME AND ACCEPT ANY AND ALL RISKS OF INJURY, PARALYSIS, OR
DEATH. I acknowledge that boating entails known and unanticipated risks which
could result in physical or emotional injury, paralysis, death or damage to me, to
property, or to third parties. The risks include but are not limited to: boat capsize;
travel in remote areas; collisions with objects or other watercraft; prolonged
exposure to cold water; hypothermia; accidental drowning; mental anguish or
trauma; illness in remote areas; exposure to sun, strong wind, cold, storms and
lightening; wrist, arm, shoulder and/or back injuries; slips and falls while hiking;
and rapidly changing adverse weather and water conditions.
I affirm that I am / my child or ward is in adequate physical health and
condition to participate in this activity. I am/my child or ward is sufficiently
physically trained and have / has the skills necessary to safely participate
in this event.

Rental Agreement
Rentals are due back no later than on the same rental date and or agreed
upon time. I understand that due to unforeseen inclement weather, I may be
required to return the rented equipment IMMEDIATELY as required by Born
To Be Wild Adventures Tours & Rentals.
I am responsible for the return of this equipment in the same state in which
I received it. I agree to pay in full for any repairs or replacement if the
equipment is damaged or lost, and/or a retrieval fee if Born To Be Wild
Adventures Tours & Rentals has to retrieve any rented equipment. I further
agree to make every reasonable effort to return all equipment to point-of-
SPORT AND I AM AWARE OF THE DANGERS OF KAYAKING/CANOEING?
SUPing. I UNDERSTAND THAT BORN TO BE WILD ADVENTURES TOURS &
RENTALS IS IN NO WAY RESPONSIBLE FOR MY SAFETY OR THE SAFETY
sale.
I AM AWARE THAT KAYAKING/CANOEING/SUPing IS A DANGEROUS
OF THE EQUIPMENT I AM RENTING.
I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS
